HOW DO I MAKE AN APPOINTMENT?

To make an appointment, call the ID Clinic nurses' station at 504-903-6959 or 903-6572.

Patient Registration Information
Please provide the following:

To verify identity and residency:
Either a Louisiana driver’s license, Identification card, or Alien registration card and current mail dated within the last 12 months in the client’s name or a current Voter Registration card

To verify income, at least one of the following for client AND spouse:
Social Security Award letter
Pension statement
Three most recent check stubs
Current Federal or State Income Tax Return (1040)
Written statement from employer on letterhead stationery
Copy of unemployment benefits check
Verification of Workers’ Compensation Income

If applicable, other documents that verify support should be provided, for example, a current food stamp printout, verification of child support income, etc.

Also required:
Copy of Social Security card of client, spouse AND dependents (Social Security statement is acceptable until the Social Security card is received)
Medicaid/Medicare card, if applicable
Commercial Insurance card, if applicable

Other items, if applicable:
Medical bills in last 12 months (paid & unpaid)
Final divorce decree
Student ID for current semester & verification of income (grants, loans, work study, etc.)
Termination letter from employer

Free Care Process:
If applying for “free care”, an account specialist will assist the client with the application process. Clients will be informed of their eligibility for free care within 10 days (or less) of completing the application. 

*A notary public is available free of charge Wednesday and Friday from 9:30 am until 12:00 pm at HOP.
